AD and Exchange on Same Computer

Hi,

I have AD (Win Serv. 2000) and Exchange 2000 on same system now I want to
migrate on to win 2003 std. with Exchange 2003 std. on new hardware. I have
done alot searches on net & found some articles on microsoft & other sites.

Can anyone suggest me the exact procedure to migrate on the new
hardware without any failure.
