I have an Actiontec PCI modem that Windows XP

Hello

well maybe the problem is the mode itself, has it worked
at all, also maybe try a different brand of modem

This is a default that windows chooses when the device is
not plug and play. you should be able to manually update
the name and force windows to recognize the new drivers
and name of device. if the modem works, leave as is.

I have the same modem and its by actiontek but modem chipset is lucent tech.
so it shows LT. It's also pnp unless you're install an older 28.8 or lower.
It's a host dependent modem, aka winmodem. The install for it will put an
icon for settings on control panel or through the phone and modems on
control panel. It likes to grab irq19 or 21.
