XP Pro Crashes when USB device plugges in

My system completely crashes and reboots when attempting
to plug in a smart media USB adapter or flash drive.
System re-boots but re-crashes over and over until the
device is unplugged.
Also it requires resetting the computer to the last know
good configuration to allow the system to reboot even
after the device is unplugged. HELP!
