wrong app brought to forground when selecting from Taskbar

Not sure if anyone else has seen this, but whenever I
choose an item from the Taskbar, that item is not brought
to the foreground - randomly, something else I have open
is shown. Anyone seen anything like this? Or more
importantly, does anyone have any ideas for how to
troublshoot something like this?

I'm trying to determine under what conditions this
occurs. I just checked again and now all the correct
items open up. It must have something to do with either
the number of open items or the type of one/some of
them. When I determine more specifics, I'll post to this
thread again.
