Word forms

I need to make Online Forms using Word. What I can't
decide is, if I should use the "Forms" Toolbar or the
Control Toolbox(VBA Forms).

The Fields made using the "Forms Toolbar" don't seem as
nice as the ones available with the Control Toolbox.

I don't need to send the forms like a webform but the user
should be able to fill in the form (including comboboxes
etc), save the contents, and attach to an email.

I would suggest either the forms toolbar or a userform.
