Word 2003 filename and path menu selection appearance

Word 2003 - menu Insert>AutoText>Header/Footer used to included selection "filename and path" - gone. What affects these menu appearances? How do I get it back?

I don't know how you lost yours, but to get it back you can recreate the
entry. Open a new blank document, select View>Headers and Footers. Enter
{ Filename \p } (use CTRL+F9 to enter the field code brackets { }). Right
click the fieldcode and select update field. The field code will change to
document1. Select the text document1 and press Alt+F3. Name the AutoText
entry Filename and path.

All done.
