Word 2003 Drop Down fields disappear in Outlook

I've created a form in Word 2003 that has several drop down selections and
check boxes.

When I email the file from word using "Mail Recipeint" all of the drop down
values and check boxes disappear when the document is sent.
This does not apply to the Text Form Field.

Is there a way to fix this?

No, that functionality is not supported in an email. InfoPath is a more
suitable application for this and allows you to email such forms. Otherwise
send the mail document as an attachment.
