windows xp shutdown has to be clicked twice to actually shutdown

any clues to this one?

Wifes computer, all updated and virus checked and adaware frequently.
Runs well. no software problems.

When start-shutdown the computer there is some activity and then
windows and that user still is in operation. can still use programs
etc under the logged on user. To shut down must do start-shutdown
again and then shuts down all ok.. I noticed that a program installed
recently (which works well) wanted to restart the computer. It also
tried to restart the computer but nothing happened after a few seconds
and just kept running ok./ To restart the computer we had to do it
manually.

Any clues??
