I wish to create a custom icon from a .bmp image - how
can this be accomplished?
 
Do a Google search for "custom icons," because you need a program to create
them and merge them into Windows. Some of these programs are free.

Make or resize the bmp file to an appropriate one (standard icons are
32x32, but you may be displaying them at a different size, eg I use
40x40, and it is best if they match). Save as a .bmp and rename as an
..ico

As cimex says, use a program created for this purpose. I use
IconWorkshop.
Alex Nichol's post is incorrect, you can not turn a .bmp image into an
icon by just changing the file extension.
