Why is my webpage listed as index.htm in the search page?

When I type my website address in the Yahoo Search Engine Page. My pages
are listed as index.htm for the home page and New Page 0 for my linked pages.
What is causing this? Is there some type of code missing from my web pages?
Is it my web server?

Did you add Titles to your pages when you created them?

As in right click: Page properties | Title.
