Why does text sometimes disappear when I rewrite a sentence?

When I insert text by typing (not cutting and pasting) sometimes the old text
gets overwritten. Is there an option in the toolbar that I can turn off to
stop this problem? it does not happen all the time.
 
At the bottom where it says the page number etc. in the middle there are four
grayed out abbreviated options. They are REC, TRK, EXT, and OVR. Make sure
the OVR is grayed out. If you double click it or you press the insert button
