Why does print preview cut off text for large custom paper?

I have created an 11x16 custom paper in Page Setup and when I preview my
work, it cuts off the top two inches and at the 11 inch mark on the
horizontal.
 
In all probability the selected printer _cannot_ handle that paper size.
Print Preview shows you what _will_ print, and the content will be cropped is
what isn't showing up.
