What is OT2004 ? I got an error msg about it

What is OT2004?
When I tried to use the Access 2003 wizzard to create a form I got a message
that it...

" couldn't find a valid source for OT2004 "

Can anybody let me know what this is and what I should do about it.

thanks for any help.
 
AFAIK, there's nothing in Access with that name. Might it be a table or
field name in your database?
