What is it called

Howdy all
I would like to add a link to my site to open a page that a customer can
enter information I require that will be sent to my e-mail address or Host.

But I don't not no off the top of head what they are called. So I can go to
the Help Section and type in "How to create a thing-a-ma-gig"

If you online site is hosted on a server that supports the FP Server Extensions and Email Transport (check w/ your host) and you
publish using FP http mode try the Feedback Form Template or the Form Page Wizard template (File New (More Page Templates) and
select the template page

you're probably talking about Forms.

Under the Insert menu, then go to the Forms option.
