Web Hosting Reccomendations

Hi all, I know that this question has been posted before, but I need an up to date answer.

Can you recommend any good web host companies?

My websites server has gone down...again....and I am getting pretty darn sick of it - when the server goes its not just the site i lose, but my e-mail too which I cant afford to be offline. The hosting companies website must also be on the same server because their site is offline too along with their e-mail (so no support available)!

So, reliability is top of my list of priorities!
Here are some other key things I require in my hosting:-
Minimum 500Mb+ Webspace
Minimum 5GB+ Bandwidth
Professsional control panel (such as cPanel)
Upwards of 5 MySQL databases (1 for the main site, and the remaining ones for testing/development)
PHP, MySQL, Apache, Perl, CGI etc.
Multiple POP3 email addresses
Full FTP acces (multiple accounts)
Recommended Guaranteed uptime.
Reasonable pricing!
