Want to print on paper length of 5 inches.

Using Access 2000. How is user defined page size defined.
I selected "User Defined" from Page Setup but there is no
place to define it. My paper is only 5 inches long that I
will print on (and 6.5 inches wide). The report properties
have a width option but not a length option. I have
grouping and a group header in this report.

You have to define the paper size for the printer, and then it will be
available to Access under Page Setup.
