Using Powerpoint to record test answers

Greeting to all... I hope someone can help me with the following:

I have developed a presentation that instructs employees on a particular
task. I want to include a couple of slides at the end of the presentation
to ask questions and record the employees typed or selected answers.

1 - is this possible?
2 - how?
