Using CTRL-A in Word 2007 but F9 is NOT updating fields?

Have a Word 2007 document with numerous cross-references. Using CTRL-A to
select the entire document but F9 is not updating the fields. Also cannot
RIGHT-CLICK on one of the fields to see "Update fields" option.

Possibly the fields have been unlinked, or they could be locked. If the
former, they are no longer fields and can't be update. If the latter, unlink
them using Ctrl+Shift+F11.
