User getting locked out on login

Is she using Terminal Services, may have an old session
still active on the Terminal Server, and has subsequently
changed her password, thus the old seesion is locking the
account out by submitting the old password, have had this
happen on two occassions to me recently.

Nope, she hasn't ever used Terminal Server, and we've ruled out any form of
intrusion, as the problem can be consistently replicated just by logging
into or out of the system on any workstation.

After consulting some other sources, we've determined that this problem may
not be feasible to try and repair (if even possible), so we're going to
backup her profile and mail store, and remove the account, and then create a
new one (which has proven itself to work in trial runs) and re-import all
her stuff.

Very strange, indeed.
I've heard of several people encountering this problem, and none have found
a resolution outside of destroying the account in AD.

If anyone encounters this problem and finds a resolution to it, I'd be
interested to hear how they resolved it.
Would be useful to know if we ever encounter a problem like this again.
