Use iCal file for new Outlook meeting request?

Does anyone know the exact fields required to build an iCalendar file
(.ics) to create a *new* meeting request in Outlook? I have been able
to create an iCal file that opens a new window to "Send updates" for a
meeting in Outlook 2007, but not just "Send" (i.e. a totally new
meeting). I'm not sure why Outlook thinks this is a pre-existing
meeting because it's not.

Also, this same file does not work in Outlook 2000 or Outlook 2003;
the attendees to not show up. What modifications to the .ics file will
make the attendees show up?
