Use check boxes without restricting other editing

Within a word document, I need to turn on protection so that the X appears on
the box once checked, but when I do that I can't use other parts of the form
that require individuals to respond to such things as information requests
 
Form field check boxes require the document to be protected. Either insert
text form fields to accept the user input or see
