Update TOC - no dialog to choose only page numbers?

F9 in TOC doesn't display Update Table of Contents dialog box to choose
update age numbers only option.

Office 2003. Where did the dialog go?
 
Sometimes you see it, sometimes not. For example, I have TOCs with no page
numbers, and they do not produce the dialog.
