Unrecognized\Unknown Media

I've connected a Adaptec AHA-2940U/AHA-2940UW PCI SCI
Controller and a 4mm dat (standalone) recognized as a HP
C1537A SCSI Sequential Device to a W2K Pro machine. RSM
shows the drive in Physical Locations which also has media
in it in the \Unrecognized\Unknown Media Pool, state is
Loaded,New. I cannot prepare the media get a messages
saying "The media is write protected." from Removable
Storage. The tape is "not" write protected. If I eject
the tape and reload it I get the same problem but the tape
number get increased by one. I've found the question on
the web, but no answer. Any help would be appreciated.

Every time I have seen this behavior on a C1537A, it has been due to
faulty firmware, or a hardware failure, on the tape drive.
