Unbound Field in Query

I am using a form for teachers to input their teacher number and then a grade
range.

The first value from the form is used in the query to limit the records to
just those with = teacher numbers the second value is used in the same query
to select a grade range, but I get an error message, You Canceled the
previous Operation, when I try to open the query.

Any idea how to solve this?
