Two contacts lists in address book

I have two contacts under address book (Outlook - Tools - Address Book)

When I go tools - options the bottom box only displays 1 contacts (I have
already deleted the duplicate) yet the top two boxes, show this address list
first and Keep Addresses In still have two contacts - how do I delete these.
Also, the right hand box (Show Names From The) also has two contacts.
 
You failed to post your Outlook version which is necessary to answer your
question. In some versions, you delete that reference here:
Tools > E-mail accounts > View or change existing directories or address
books > Outlook Address Book > Change.
 
Outlook 2003, I deleted here as well and I still have two contacts.

If so, you would be the first and only.
Try again.
Restart Outlook after you do.

Restarting Outlook did the trick
