trying to connect a form to a database on the network

I am trying to connect a form that I have in outlook to a
database that I am using on my network called Millenium.
I want to be able to type names into this form and then
pull out other information from the database, based on
those names. What code would I need to be able to make
this happen? I just don't know how to make the connection.
