Troubleshooting a form

I have created a form using FrontPage 2000, and posted it on my web
site. A number of people have submitted information with it, but of
course one person has e-mailed me saying "Your form does not work." I
asked him what happens when he tries, but have not received a reply.
What questions should I be asking, and are there any tips for
troubleshooting problems with a form?

If it works for others, and not him...

1) Get him to tell you the definition of "does not work", including what he
is doing when he tries to submit.
2) What O/S and browser is he using on his computer?
3) Does he have a firewall configuration that might be stopping it? Or, a
configuration in the browser?
