TOC for multiple documents

I tried to post this about a half-hour ago, and it hasn't shown up yet. If
it ends up posted twice, I apologize.

I've been assigned to revise a large (900+ pages) document that consists of
about twenty chapters. The front matter and chapters are all separate
documents. The page numbering in each chapter starts over from 1 and includes
the chapter number (e.g., 3-1, 3-2, etc.).

I need to include a table of contents, table of figures, and list of tables
in the front matter that includes both chapter and page numbers. Is there an
easy way to generate these tables?

I'm using Word 2003 and Windows XP Professional (SP2).
