TOC can't determine page numbers

I am running Word 2002 on Windows XP. Endless problems.
Example: I have a document with five sections. Each
section has a unique header, but does not restart page
numbering. The page number is in the header. I create a
new section at the beginning of the document and put a
style-based TOC in it. All page numbers in the TOC are
0. I try F9 to get it to reset. No luck.

Also, the TOC ignores my paragraph commands for right
indentation, and runs into the margins. I've tried
putting it inside an invisible table. This doesn't help.

If your headings are (only) in the header, then they can't be used to
generate a TOC. Because header text appears on every page, the TOC wouldn't
know what page number to use. There are other approaches, such as using a
StyleRef field in the header to pick up your heading text. Or you could put
a TC field in the body of the document to include the header text, then base
the TOC on TC fields.

I am using styles to generate the TOC. The texts to which
those styles are applied are in the body, not the header.

Example:
Section 1 header contains:
"Acme Proposal" in style "Document Title"
"Introduction" in style "Section Title"
Date
Page number

Section 1 body has 3 pages.
Page 1 of Section 1 contains the text "Introduction" in
style "Heading 1", text "Current situation" in
style "Heading 2", "Region: US" in style "Heading 3".
Page 2 of Section 1 contains the text "Region: EU" in
style "Heading 3". And so on.

Section 2 header contains:
"Acme Proposal" in style "Document Title"
"Assets Inventory" in style "Section Title"
Date
Page number

Section 2 body has 17 pages.
Page 1 of Section 1 contains the text "Assets Inventory"
in style "Heading 1", text "Region: US" in style "Heading
2", text "9 CFR" in style "Heading 3". And so on.

I insert a TOC at the beginning of the document. It picks
up all the text in styles "Heading 1", "Heading 2",
and "Heading 3". All the page numbers are 0.

Introduction............0
..Current Situation......0
...Region: US............0
...Region: EU............0
Assets Inventory........0
..Region: US.............0
...9 CFR.................0
