TOC Assistance

I've created a TOC that spans across multiple pages. On the first page I
inserted the heading Table of Contents and column headings (i.e., Section,
Page, etc...). How can I get the heading and column headings to appear on
the ensuing pages (i.e., Table of Contents Cont.)?

Insert a section break after the TOC; then in the first section header enter
a conditional field - something like

{ IF{ Page } = 1 "Contents" "Contents continued ." }

Put the TOC in a separate section and put the headings in the page header
(along with any other desired content).
