Thousands of Copies of One Message in Sent Folder!

Outlook 2003, Exchange 2003.

One user has thousands of copies of a sent mail in her sent items. This has
occured several times for this user, sending to a few different internal
addresses. It doesn't happen for every message and isn't reproducable that
I have found.

I've turned off cached mode and restarted Outlook, it had no effect.

Does the recipient receive multiple messages?
What add-ins are in Outlook?
Do you see multiple copies of the sent item in OWA?
