text boxes in templates

Can anyone point me to an informative website where I can learn to do the
following:

I wish to create a template email where the 'To:' field contains my own
email address. The body of the email needs to be some text boxes with labels.
I want to send this email to my clients / install it in their outlook, so
when they click on it they can fill some information in the text boxes and
click send.

I should recieve the email with the text boxes filled.

I have tried to do this, but all text in the text boxes gets lost.

Outlook doesn't support this. A survey web site might be a better solution.
