System exception access-violation (0cC0000005) on WXP SP2

Hi,
Im trying to identify an error received from application:
"System exception access-violation (0XC0000005) ocurred at address
0X7C921E58. Attempted to 'read' data at address 0XFF30FCFF
Since Exception Origin and Last Throw are maningless for this exception,
determine the origin from the address and the Map file"

The question is:
How can I identify what is happening with the SO when this error occured?
How can I interpret this message? Does exist any tool from microsoft to track
this error?
