Survey says... Check boxes in an email?

I would like to send a simple survey (mark all that apply) to several
thousand hits in my database. Check boxes for "all that apply" in a list of
seven items. And, maybe a comment box or two? No not need any statictics or
fancy analysis. Just a reply email with some boxes checked and maybe a
comment.

Sending that sort of thing over the Internet can be complicated and often
fail depending on how you do it and if Outlook is used at both ends and if
the mail format remains untouched in the transfer. The custom form would
also need to be published in each recipient's Outlook for this to work.

The easiest two ways to make this work are to design the form as an HTML
document or to send it as a Word or HTML attachment. Even with the HTML
document it may not work as expected however.
