Subreport date range issue

I have a database to track employees production, quality, and calls.
Employee names, etc are tracked in one table, calls are in another table, and
quality & production are in a third (I used the call tracking template
included with access and added the quality/production table). I created a
report for the production & quality and inserted it into the weekly call
summary report. Everything reports correctly, however, the date range form
opens four times before the report is displayed. How can I get this to only
open once?

Please disregard this message. The problems got even worse when data for a
second employee were added.

To let me better understand the issue and perform effective reasearch,
please help collect the following information and send them to me
(changliw_at_microsoft_dot_com):
1. Report file
2. A test database for providing sample data

If the files are large and not proper for email delivery, please first send
me an email and then I will let you know how to upload large files to me.
