SQL Username/Password Prompt Doesn't Appear

I have an Access 2003 .ADP project tied to an SQL Server database that
I created using the Upsizing Wizard. What do I have to do in order for
the user to get prompted for SQL Server authentication?

The "blank password" checkbox in the File|Connection dialog does not
seem to work because it won't allow me to leave the dialog without
supplying a password. I want the user to provide an SQL
username/password everytime they launch this ADP rather than it being
saved.

How do I do this?

P.S. I want to use username/password authentication (not NT integrated
security).
