How do I stop this program, which otherwise seems fine
stopping Sophos adding itself back to the startup program
after it has updated? If I tell it to allow (or for that
matter block) it seems to take no notice.

Have you been in touch with Sophos support? Checked out their web site for
suggestions?

I don't have a fix for this, but it may help for the vendor to hear from
you--so they know about the issue and that their customers are impacted. In
this case, I suspect that's already true, but perhaps they have a
workaround.
