ServeRAID 3H failure?

I have an IBM Xseries 230 (8658) and I've recently bought a new ServeRAID 3H
controller. I upgraded BIOS/Firmware from the ServeRAID Support CD v7.12.
Everything worked fine for a few days, but know the controller sees a hard
drive with SCSI ID 3 always, even if drive isn't plugged in. At first, I
thought it was a problem with backplane but with the on board Adaptec
controller everything is fine. Is it a pemanent controller failure or is it
posibble to fix it by downgrading BIOS/Firmware for example?

To clear this, you may have to delete the array and reset the controller and
drives to factory defaults. I just recently had a Netfinity 4500r with a 3x
controller and it was seeing 5 extra devices in the POST but any other time
is did not report these devices. The drives (3x73GB / Raid5) would go
defunct and would have to reboot to serveraid utility to bring drive online.
This happened a number of times over a 1 week period. It finally defuncted
2 of the three drives and all was lost then. I then re-loaded the firmware
and bios from the serveraid cd and deleted the array. After that I rebooted
and hit CTRL-I and went into the mini config. Under advanced reset to
factory defaults with the drives attached. This will reset the controller
and delete all info from the drives. I then rebooted to the serveraid cd
and the extra devices were gone. Did have to re-install Windows and restore
the data and since then the phantom extra drives are gone and the drives are
not going defunct.
