Usually believe it or not, it just user error. You accidentally touch the
mouse button, accidentally drag things into other folders, etc.... I see it
all the time.
For yours I would first check your mouse settings and make sure there's
nothing in there that is set that makes it act as you describe. Also I would
try updating your video card drivers and mouse/touchpad drivers from the
makers website.

One thing that helps is to use TweakUI, Mouse page, to put up the value
of drag sensitivity to say 20. That way it is much more difficult for a
small jitter of the hand to be interpreted as a drag - and a real drag
will be going further than 20 pixels
