Scanning An Existing Form

It is possible to scan an existing form and then create a database with the
fields?

Can you provide more details? A form generally displays data and provides
access to underlying tables...but not usually ALL of the tables in a db.
 
I would expect it to be easier to just start with the paper copy of the
form and an understanding of what the data consist of and what you are
expecting the database to do and then manually create the database.
