RPC over HTTP dialog missing

I have configured my backend Exchange 2003 server for RPC
over HTTP, but in Outlook 2003 the dialog on the
Connections tab does not display the Connect to Exchange
over the Internet option. I know I have seen it at some
point, but that could have been in XP.

Can anyone tell me how to get to this option, as it is
listed in all the KBs and documentation included with
E2k3.

Which Patch? I keep up to date on anything coming
through Windows Update and OfficeUpdate.
