Replacing forms

Here is a question I know I should be able to find on the net but I have
failed. Tried the Sue Mosher book and MSKB. (Used to be a site called
something like 'slipshot' but I can't find it.)
In Outlook 2002, how do I change all existing forms (in this case,
Contacts) to an existing custom form?
