Remove document "lock"...even though it is not locked

How do I remove what appears to be a document "lock" for MS Word. Although
my document is not currently being edited, it appears to be. I have
re-booted, restarted, etc., nothing works. Also, the Custom and Summary tabs
are now missing in the Properties section/tab.
 
Does this happen to just this document or others as well?
