Quick Launch toolbar problems

I'm having two problems with my Quick Launch toolbar under Windows XP
(Home). Every now and then -- for reasons I have not yet been able to
determine -- my Quick Launch toolbar with either disappear completely (and
will still be gone, even after a reboot), or the icons will have gotten
"shuffled around" in the toolbar.

If the toolbar disappears completely, right-clicking on the Task Bar and
selecting Toolbars shows that it is no longer checked. I can re-check it,
and it will once again appear. But why is it disappearing in the first
place???

Then, after I've re-checked it (or sometimes even when it does NOT
disappear), I find that the icons have been moved around. Most of the time
(but NOT ALWAYS), they have been restored to a purely alphabetical order.
(That's usually when I've "lost" the toolbar, and had to go back and
re-select it.) However, sometimes it's just a random order (which is more
often the case when the toolbar never disappeared, but just got "shaken
up").

Any ideas what would cause this? (BTW, I also have periodic -- like, 3-4
times a day! -- crashes of Windows Explorer. I'll just get the message that
Explorer has to shut down, and do I want to Send a message to Microsoft.
SOMETIMES this is coincidental with the toolbar then disappearing -- but I
don't have enough empirical data to *prove* that it's related.)
