Quick Launch (and explorer double clicks) freeze

I've developed a problem with opening programs from the Quick Launch bar,
and also when double clicking files in exporer to open their relevent
programs.
It's an intermittent problem that has started happening more often than not,
but still sometimes works properly. However now it's happening about 80% of
the time.

When clicking on a program in the quick launch, it appears to crash the
whole taskbar and start menu.. Nothing happens, and the mouse turns to an
hourglass when passing over the taskbar - nothing else can be pressed in it.
However, after about 10 minutes if left alone, the program I tried to start,
correctly starts, and everything returns to normal.

The same happens when double clicking a file in explorer. The explorer
session appears to freeze until the program correctly opens up about 5-10
minutes later. Only the explorer session used to open the file freezes.. I
can sill use the taskbar, and open up new explorer sessions.

This problem also occurs when pressing the quick launch buttons on my MS
Wireless Desktop Elite Keyboard using the latest Intellikey drivers (this
did NOT start happening after updating the drivers - been using these
drivers without any issue for some time).

When the "freeze" is occuring, I've opened up the task manager to see if
there's anything out of the ordinary, but there isn't.. No programs appear
to be taking up the CPU (system idle shows 98-100% CPU).
Event viewer shows nothing unusual either.

I'm fairly careful with this system as it's my business PC.. I rarely
install new software on it, run a nightly defrag with Diskeeper, have McAfee
monitoring for viruses 24/7 (and doing a scan every 2 days), and update the
system via Windows update immediately whenever new updates are ready.

Any ideas what may be causing this?

1. Don't depend on McAfee alone.
2. Same goes for new updates. Not all are meant to be installed on every
system.

Tip: Download the four cleaners mentioned here, keep them updated and run
them often:
