Problem generating a TOC - Help!?

Love the hyperlinked TOCs for long documents (current one is 100 pp), BUT ...
I continue to have a problem. I complete the document and insert the TOC,
click to generate it. Half the document comes up as headers in the TOC; I
have no idea why; and half is not labeled at all. So, I just Select All to
make the whole document Normal (or Body Text) and then manually select the
TOC levels.

That's fine... what's NOT fine is that when I make it all body or normal
text, I lose all my bullets, all my formatting... on 100 pages!

Please, can anybody out there help me to do this right??

Thanks much --

OK, OK, I learned to type on a typewriter, see?, and Styles were not a
problem back then. NOW they are. I'll learn styles. In the meantime, though,
must I go back and reformat everything? And when I do, do I have to attend to
some style sneaking in unwanted??

Disable "Define styles based on your formatting." Then display the Document
Map. Everything that appears in it is formatted as a heading. Click on any
"heading" that shouldn't be one; that will take you to it in the document.
Apply some more appropriate style. Repeat as needed. When you're done,
update the TOC.
