The easiest way is through the header/footer (Menu item View). Click on the
Insert Auto Text button and choose what you want to include.
 
Insert Author and CreateDate fields in one of the following ways:

1. Insert | AutoText | Header/Footer: Created by, Created on

2. Insert | Field: Author, CreateDate

3. Ctrl+F9 to insert field delimiters and type Author or CreateDate between
the braces. F9 to update.
