prevent password request when visitors click submit

When visitors click the submit button on my web page it asks for username and
password. I need visitors to be able to submit survey information anonymously
without signing up with a password.

Make sure the _private folder has "server write" permissions set.

I found out that there was a line of code that frontpage produces that was
not compatabe with my servers form handler. I had to relplace that line with
my web sites servers' (pre-installed forms handler) & add (Specify Recipient
E-mail address)
