popup window requesting info/feedback then never shows again, how?

I have a pop up window that opens up asking for feedback and want to stop it
from showing every time they visit after they have filled it in first.

I have them select the area they live and their name and age, just for
statistics. then when they visit again it should not pop up again and it
then greets them using there name they entered

I have once had a script to stop the pop up showing more than 3 times, but
can not find it now. where can I get this one or another script to do it.
