Pop up screen when opening outlook looking for LDAP

I just updated my outlook and now I get a pop up window that is looking for
the LDAP.
There is server on the system, but it does not have a mail server running.
This is the only computer that is having this problem.

On the screen is has a text box with
Name Server: Null
Port 3268
Username:Administrator
Password: (this is blank)
and a checkbox for SPA

if you just click ok.. it goes away... but it is irritating the user. Has
anyone had this problem

You can remove the LDAP address book via Tool > Email Accounts > View/Change
existing directories and address books.
